The symptoms of this most common problem can be described and observed in many ways: gain changes with increasing common mode signals at the input, when V1 and V2 get too high, the output signal gets smaller or raising signals at the input makes the output signal go down. In fact, this is a very common complaint when engineers try to use an in-amp on a signal supply voltage.
